Job Description
We are hiring reliable LPNs to make high-volume, scheduled follow-up calls to recently discharged patients. No beside work - just compassionate phone-based care.
RESPONSIBILITIES:
· Call patients post-discharge using structured scripts.
· Reinforce instructions & check on medication compliance.
· Escalate concerns to provider.
· Document in EMR (training provided).
REQUIREMENTS:
· Active LPN/LVN license (unrestricted).
· 1+ year nursing experience in clinical setting.
· Tech-savvy, strong phone communication.
· Reliable internet + HIPAA-compliant workspace.
