Responsibilities
Heritage Home Health nurses work with patients, their families, and other healthcare professionals in the patients place of residence. In addition to performing visits and completing coordination of care, nurses are the point of contact for all disciplines involved with providing care to the patients. Nurses follow agency, state, and federal policies and procedures. Nurses will consult with the patients primary care physician, collect OASIS information (Home Health only; RNs only), ensure medication profiles are accurate, and attend meetings as required.
